Though I have handled guns since the age of nine, the only formal training (if you can call it that) is listed below.

USAF Basic Training
Indiana Army National Guard
USAF Reserves

Future plans are to attend some classes held by Bragg, Burkett, Max or Travis for USPSA competition.

So far I have only had one "serious" class.

Strike Tactical, 2-day pistol and rifle class. (all close quarters drills)

This was also Henk Iversons first all "civy" class.

I only started keeping track in late 2002, but the list is a little long to post here. I have over 360 hours of formal instruction (i.e. "paid") in various defensive shooting and tactics-related topics. I've had more informal instruction than I can remember. Among others, I have studied with Ken Campbell, Louis Awerbuck, Henk Iverson, Tony Barrera (aka Doc Gunn), and most recently Rob Pincus. I've completed a lot of classes with pistol, carbine, tactical medicine, and a few other things here and there. In addition to continuing those studies, I intend to branch into long range precision rifle shooting soon.

Here is a snippet of some classes I have completed from my spreadsheet:

Introduction to the Defensive Pistol Ken Campbell
Tactical Handgun - Stage I Louis Awerbuck
Tactical Rifle Louis Awerbuck
Tactical Rifle Louis Awerbuck
Tactical Handgun - Stage II Louis Awerbuck
Tactical Carbine Operator Course Xxx Xxxxxx
Tactical Carbine/Handgun Louis Awerbuck
Tactical Handgun - Stage II Louis Awerbuck
High Intensity Tactical Training (HITT) Louis Awerbuck
Tactical Treatment of Gunshot Wounds Tony Barrera, MD
Problem Solver Louis Awerbuck
High Intensity Tactical Training (HITT) Louis Awerbuck
Tactical Carbine/Handgun Louis Awerbuck
Gunsite 123 (off site Indiana 3-day carbine) Ken Campbell
Tactical Treatment of Gunshot Wounds Tony Barrera, MD
Tactical Carbine/Handgun Louis Awerbuck
Handgun Level I Henk Iverson
Carbine Level I Henk Iverson
Combat Focus Pistol Rob Pincus

In addition to continuing with the instructors above as long as I can, I would also like to patronize the following schools/instructors (in no particular order):
David Elderton (the Ky-Tac guy)
Clint Smith
TJ Pilling of Tiger Valley in Texas
The Badlands in Oklahoma
TDI in Ohio
Massad Ayoob
Randy Cain
Kyle Lamb of Viking Tactics
Tom Givens of Rangemaster
FIRE Institute in Pennsylvania
Hany Mahmoud and Steve Moses in Texas

I've also been shooting in combat/action shooting games for over 13 years. Some dismiss the defensive training benefits of doing so, but they are excellent tools for improving marksmanship, gunhandling, movement, and a few other defensive skills while under external pressure of a clock and an audience. The gun games are also an excellent testing ground for your weapons and how you carry and use them.
