Form I Question

Filling out my Form I for SBR. Very simple, but I've bumped into a minor question, and don't want any hick-ups.
Question 4a; asks for original manufactures location. Instructions say address.
Is city, and State, enough? Or do I have to fit in the street address too?
Typing it on the PC only gives me 1 line and the complete address won't fit.

City and State will suffice.

I put name, city and state. Example, JOHN DOE CO. ANYWHERE, IN.

Use the factory roll marked address.

Example: If using a STAG lower the roll mark reads as follows.
STAG ARMS
New Britain, CT
USA

Most all AR lowers are marked this way with the original makers info.
