It has been kept in a Speck case the whole time I have owned it.
Comes with box and everything that came with it. (USB cord, wall plugin, manuals, earbud headphones).
The phone is my camera, so pictures will not be added until I am at work tomorrow and can use a camera there. I will edit the post then.
