Where can I get a custom nametape?

I got a new hat, and am looking at getting a custom nametape for the back of it. I'm pretty well known as "Double T" and would like to get that in a 4" tape in Kryptek for the back of my new hat. Anyone know of a good online dealer that they've dealt with for something like this? I know milspec monkey is spot on for patches, but I don't think they do any custom name tapes.

US Cavalry carries them. I've had good service with other items, but never bought name tapes.

You might also try a local sports apparel/embroidery shop. I had two made for $5 at a local vendor near work.
