Some of the locations for free narcan almost seem counterintuitive, like the entrance to ER or the entrance to a county jail. Also, at least one county was using the effective use of narcan to file felony charges (or at least used to). The attached article (ca. 2020) points to weak Good Samaritan laws in Indiana as another factor in some deaths.
